Horton was said to be seeking a second opinion on his ailing elbow after his MRI results came back concerning, but he will ultimately undergo what will likely be Tommy John surgery. The 24-year-old posted a 2.45 ERA in 7 1/3 innings this season after breaking out with a stellar 2.67 ERA in 118 innings for the Cubs in 2025.

Not good news for Horton, for the Cubs, and for fantasy owners. Horton exited with forearm discomfort after one inning in his last start against Cleveland last Friday and was subsequently placed on the 15-day injured list. However, initial MRI results came back unfavorable and Horton is scheduled to visit Dr. Meister for a second opinion. The young Cubs righthander finished second to Atlanta's Drake Baldwin in last year's National League Rookie of the Year voting after going 11-4 with a 2.67 ERA. On the surface, it sounds like Horton is heading towards a long-term injury and possibly surgery, but more will be known after his second opinion on Tuesday. The Cubs have not announced who will take Horton's spot in the rotation on Wednesday, but Colin Rea is a likely option.
