Milano went down with a knee injury of some kind in the first quarter and had to be carted off the field. He's already been ruled out, and many signs point to this being a more serious injury than fans would hope. He'll be out for the rest of the game and possibly more time beyond that, which is a devastating blow for Buffalo's defense, who has already lost Tre'Davious White (Achilles) for the rest of the season.
